I/We, the parents/guardians of the before mentioned candidate(s) for a position on the Kingsgate Monarchs Swim Team, approve his/her participation in any and all Kingsgate Monarchs Swim Team activities and recognize the possibility of physical injury incidental to the candidate’s participation in swimming and assume such risks and hazards. However, I do not assume the risks of negligence in transporting the candidate(s). I hereby release, discharge and otherwise indemnify the Kingsgate 3 & 4 Monarchs Swim Team, organizers, sponsors, supervisors and participants for any claims we parents may have arising out of any injury to the candidate(s), unless caused by gross negligence. I reserve any claims for negligence we may have against the individual transporting the candidate(s). I recognize this Hold Harmless Agreement does not release any claims the child may have for negligence in his/her own right.
I/We, the parents/guardians of the before-named individual(s) give my/our permission to the Officers and Coaches of the Kingsgate Monarchs Swim Team to have my/our child(ren) transported to Evergreen General Hospital or to any hospital in the immediate area where an injury or illness has occurred.
